Why was I billed twice or overcharged (even if the transaction was rejected)?

We've noticed this issue has popped up a few times but without exception it was always automatically cleared with the customers bank within 3-5 days. It usually only occurs whenever an order is changed post purchase or if an attempted credit card purchase is declined. The customer is not actually being billed twice just the bank perceives a changed order as possibly two transanctions and is usually slow to void the first and/or declined attempts.
